A 48-year-old man was admitted to our hospital because of a mass in his mid-line upper abdomen. He had no past history of abdominal surgery or trauma, and was morbidly obese (body height 138cm; body weight 81kg). On physical examination, fist-sized mass was palpable in the midline of the upper abdomen. Abdominal computed tomography scan showed the fat tissue protruded outside of the peritoneal cavity. We performed an operation based on a diagnosis of linea alba hernia. There was a defect about 4cm in diameter in the linea alba. The hernia sac was dissected out, and then we repaired the hernia using Bard Composix Kugel Hernia Patch^®. The postoperative course was favorable, and there have been no signs of recurrence up to now. In this case, the hernia may well have occurred in combination with the fragility of the linea alba and abdominal high pressure due to the morbid obesity. The technique using Bard Composix Kugel Hernia Patch^® reduced postoperative pain and the tension of the abdominal wall. It is useful for a huge hernia such as in this case.
