Abstract

10歳男児。初診2ヵ月前より左下肢に皮疹が出現した。初診時,左下肢に浸潤のない暗紅褐色から暗紫紅色の斑が多発していた。組織学的には真皮上層の血管周囲に軽度の単核球浸潤と出血を認めた。慢性色素性紫斑の小児例,さらには片側性は稀である。自験例は慢性色素性紫斑の一型ともされるlichen aureusあるいはlichen purpuricusと類似した所見を呈していたが,臨床組織学的所見より鑑別し,広義の慢性色素性紫斑と考えた。

A 10-year-old boy had a 2 month history of asymptomatic, reddish-brown patches on his left leg. The histological finding showed a superficial perivascular lymphocytic infiltrate with extravasation of the erythrocytes. Such a unilateral case is rare, but these findings are consistent with the diagnosis of chronic pigmentary purpura. Though lichen purpuricus which is considered to be one of the types of chronic pigmentary purpura, often occur unilaterally, a differential diagnosis can normally be made based on the clinical and histological findings.
