Abstract

78歳女性。皮膚に初発し,鼻腔に再発をしたnasal-type NK/T-cell lymphomaの1例を報告した。初診の約3ヵ月前に,左肘部に暗赤色の腫瘤が出現し急速に増大した。初診時直径3cmの腫瘍の皮膚生検組織像では,腫瘍細胞は真皮全層にびまん性に認められ,表皮内にも胞巣状に浸潤していた。免疫組織学的には腫瘍細胞はCD2, CD3, CD56に陽性であった。また<I>in situ</I> hybridization法にて腫瘍細胞にEBウイルスの感染が確認された。全身検索にて他臓器病変はなく,放射線療法が著効した。初診の2ヵ月後鼻腔に再発を認めたが,同じく放射線療法にて病変は消退した。その後も皮膚に3度の再発があったものの,いずれも放射線治療に反応し,初診後2年の現在,ほぼ寛解の状態である。

A 78-year-old woman with nasal type NK/T-cell lymphoma is reported. She developed dark red nodule on her left arm. A biopsy specimen taken from her left arm showed a diffuse infiltration of tumor cells in the dermis and epidermotropism was observed. Tumor cells were observed to be atypical medium or large-sized cells with convoluted nuclei. This case was positive for CD56, CD2, CD3. EBV was found in most tumor cells. She was treated with radiotherapy and thereafter her skin lesion significantly improved.
