Abstract

A case of recurrent hemorrhage from the junctional mucosa between ileal conduit and the peristomal skin was reported. Percutaneous transhepatic portography showed that the blood of the portal vein flowed from ileocolic vein via stomal varices to right femoral vein. Obliterattion with several metallic coils and ethanolamine oleate was performed. The local sclerotherapy of the peristomal skin was added before and after percutaneous transhepatic obliteration (PTO). The stoma and the peristomal skin were cared with ointments to reduce stimuli of stoma appliance. The patients was educated and trained not to injure the junctional mucosa in defecation. After these treatments,the patient became free from hemorrhage for 4 months. A superior mesenteric arteriography revealed that blood flow from stomal varices returned to superior mesenteric vein and got disconnected with the right femoral vein.
