Abstract

A case of adenoid cystic carcinoma of the breast diagnosed by aspiration biosy cytologg is reported, along with a review of the Japanese literature.<br> A 57-year-old woman was seen at the hospital because of a palpable tumor in the CE area of the right breast. On clinical examination there was a firm, well-circumscribed tumor with the diameter of 1.7cm. XP-mammography visualized the tumor as a relatively round mass with a slightly irregular margin, and ultrasonography showed fibroadenoma or adenosis tumor like shadow. Histological findings of aspiration biopsy cytology included globules of myxoedematous storoma surrounded by epithelial cells with little cytoplasm and small hyperchromatic nuclei, suggesting adenoid cystic carcinoma. However, the definite diagnosis of malignancy could not be made from imaging findings. An excisional biopsy was performed which confirmed the diagnosis of adenoid cystic carcinoma just before radical surgery. The patient underwent a mastectomy (Auchincloss's method), when no metastasis was detected in the dissected axillary lymph nodes. It is important to know histological classification of breast cancer in order to make the accurate diagnosis before operation. Adenoid cystic carcinoma of the breast metastasizes to lymph nodes in a low incidence compared to that of usual breast cancers and resultantly it has a good prognosis. We consider that a more limited surgery could be selected for the patient.
