We report a case of urinary excretion of enteral Gastrografin for small intestinal obstruction without intestinal perforation. A 52-year-old woman was admitted to the hospital under a diagnosis of postoperative adhesive small intestinal obstruction. We performed a gastrointestinal study with 120ml of enteral Gastrografin. Eight hours after the study, abdominal X-P films showed pooled Gastrografin in the dilated upper small intestine and opacification of the bladder. Although the urinary visualization with Gastrografin is a finding suggestive of intestinal perforation, we observed the patient conservatively because clinical findings of intestinal perforation were absent. Usually, enteral Gastrografin is not absorbed through the gastrointestinal tract and excreated with stools. However, alll clinicians need to be aware of that several conditions such as over dose of Gastrografin (over 2ml/kg), small intestinal stasis, mucosal injury and dehydration induce the absorption through the gastrointestinal tract resulting in the radiographic enhancement of the urinary tract without intestinal perforation.
