A 68-year-old Japanese man, who had undergone total gastrectomy for gastric cancer 3 months earlier, was admitted to the hospital because of appetite loss and high fever.<br> The biopsy specimen of the small intestine showed Strongyloides stercoralis and its eggs. Although 3g/day thiabendazole was administered, he died of septic multiple organ failure at the 18th hospital day.<br> Strongyloides stercoralis is often seen in Okinawa prefecture or the other Nansei Islands in Japan. It can induce fatal septic syndrome in compromised patients. This patient had not lived in Okinawa but had been treated for strongyloidiasis 15 years before admission. Even people who have not lived in rural tropical communities can carry the intestinal threadworm.<br> It is useful to examine the feces and to conduct endscopic biopsy when Strongyloides infection is suspected in a patient presented with severe diarrhea of unknown origin and malnutrition after surgery.
