THREE CASES OF ADULT INTUSSUSCEPTION CAUSED BY AN INTESTINAL LIPOMA

Abstract

Intussusception is relatively rare in adults and is commonly caused by some organic diseases. We experienced three cases of intussusception caused by a lipoma in the ileum end. Case 1: A 70-year-old man seen at the hospital because of abdominal pain and vomiting was suspected to have intussusception by abdominal CT examination and underwent ileocecal resection. A tumor 28mm in diameter was present in the ileum about 10cm proximal to the Bauhin's valve, and it was diagnosed as lipoma. Case 2: A 73-year-old woman, who had a past history of idiopathic ileus when she was 66 years old, was seen at the hospital because of symptoms of ileus including abdominal pain, nausea and vomiting. Following abdominal CT examination, intussuception caused by a lipoma was suspected. A tumor 29mm diameter was present in the ileum, 25cm proximal to the Bauhin's valve. Case 3: A 79-year-old man who was hopitalized for fracture of the left thigh showed occult blood stool. He was also suspected to have intussusception caused by a lipoma in the right lower abdomen by abdominal CT scan. Laparoscopicassisted resection was performed and the definite diagnosis of lipoma was made.<br> It is suggested that we have to consider intussusception when adult patients have nonspecific abdominal symptoms and ileus. Adult intussusceptions are often caused by lipoma, in that abdominal CT scan is very useful for diagnosis.
