A 64-year-old man complaining of a right inguinal induration was diagnosed as having advanced rectal cancer with pelvic and inguinal lymph node metastases. We considered that only surgery might result in limited outcome and conducted preoperative chemoradiotherapy: UFT 400mg/day, 5-FU 600mg/m2/24h/week (pharmacokinetic modulating chemotherapy: PMC) and hypofractionated radiotherapy: four-field box technique, 5Gy×4 fractions. Ten days after the chemoradiotherapy, we conducted an abdominoperineal resection without lateral lymph node dissection. Histological examination revealed no tumor cells in the specimen, showing a complete response (CR). He received postoperative chemotherapy with PMC+Leucovorin (250mg/m2/week). Inguinal lymph node metastasis disappeared in 3 months (CR) after the operation. CR has been maintained for 2 years 11 months after the operation. In advanced rectal cancer with multiple lymph node metastasis, preoperative chemoradiotherapy may be regarded as one of the treatment methods that could omit lateral lymph node dissection.
