Abstract

In order to clarify clinicopathological features of acute emphysematous cholecystitis, we studied consecutive five patients with the disease diagnosed by CT from January 2000 to January 2005. There were three men and two women with an average age of 63 (56-69). Chest XP could detect gallbladder gas in two cases and US in only one. CT revealed gas in the gallbladder wall in two cases, perigallbladder gas in one and bile duct gas in four. All patients received PTGBD, followed by elective surgery. Gallbladder stone was observed in all cases, and common bile duct stone in two. Cultures of bile discharge resulted in Clostridium perfringens in four cases, Esherichia coli in two and Plesiomonas shigelloides in one. Histopathological diagnosis was gangrenous cholecystitis in all cases. Since acute emphysematous cholecystitis is serious cholecystitis, rapid treatment would be required.
