Abstract

大腿部は分層植皮の採皮部として好んで選択される部位の一つである。今回我々は, 全身麻酔が困難, または全身麻酔を拒否した6例に対して外側大腿皮神経ブロックによる大腿部からの採皮を試みた。6例中5例において平均21.4×10.7cmの無痛野を大腿外側面に得ることができ, 分層採皮を行った。外側大腿皮神経ブロックは手技が非常に容易であり, 分層採皮を施行する場合に, 有用な麻酔法であると考えた。

The thigh is a common donor site for split skin grafts. We experienced regional blockade of the lateral femoral cutaneous nerve for harvesting split skin grafts from thighs in 6 patients. We blocked the lateral femoral cutaneous nerve by injecting 10 ml of 1% lidocaine with or without 1 : 100,000 epinephrine. Sufficient anesthesia was achieved in 5 patients. The remaining patient obtained partial anesthesia of local infiltration into the thigh. We suggest that the lateral femoral cutaneous nerve block is very useful for harvesting split-thickness skin grafts from the thigh.
