We report a rare case of transomental hernia. A 66-year-old man was admitted to a hospital because of sudden abdominal pain on May 13, 2005 while he was travelling. Because ileus developed, he was transported to our hospital five days later. An abdominal CT scan showed ileus, ascites, and a convergent image of mesentery vessels, and he was diagnosed as having strangulation of the ileus. An emergency operation was performed on the same day. Laparotomy revealed 950 ml of hematic ascites and a large segment of severely congested small bowel incarcerated by a hiatus of the omentum. When the incarceration was released, the color of the small bowel returned to normal and we did not resect the segment. The post operative course was uneventful except for wound infection, and the patient was discharged 36 days postoperation.
