The sign of Leser-Trélat which belong to paraneoplastic dermadrome is defined as sudden appear-ance and rapid increase in number of seborrheic keratoses. A case of advanced gastric cancer detected by the sign of Leser-Trélat is reported. A 74-year-old man visited the Department of Dermatology at a nearby hospital because of rapid proliferation of seborrheic keratosis with itching on his trunk. A diagnosis of the sign of Leser-Trélat was made, necessitating upper gastrointestinal endoscopy that indicated advanced gastric cancer. Abdominal upper CT scanning revealed the swelling of the regional lymph nodes of the stomach. He was admitted to our hospital, and total gastrectomy was performed. The histological findings were classified as poorly differentiated tubular adenocarcinoma. Seborrheic keratosis characterized by pruritus rapidly increasing size and number should lead diagnosticians to suspect the sign of Leser-Trélat and the possible existence of visceral organ malignancies.
