Abstract

全国の臨床研修指定病院638施設を対象として, 2004年10月に病院感染制御の体制や取り組みに関する有記名・自己評価記入式の調査を実施した. 質問項目は, 病院の規模や職員数, 医療機能評価機構の認定の他に, 1) 病院感染制御の組織体制, 2) 病院感染サーベイランス, 3) 感染経路別予防策, 4) 針刺し対策, 5) 病院食の衛生管理, 6) 医療廃棄物の処理, 7) カテーテル関連感染症対策, 8) 洗浄・滅菌業務, 9) 抗菌薬の使用法, 10) 手術室・ICUにおける感染制御, 11) 職業感染制御, 12) 病院感染制御に対する職員意識, 13) 病院感染制御の問題点, などであった. 446施設 (69.9%) から回答が得られた. ほとんどの病院が感染対策委員会を設置しており, 4分の3が感染対策チームを組織していたが, 感染制御担当者が感染制御の実務に充当する時間は限られていた. 医療機関が対応に苦慮している感染制御は, 「病院感染サーベイランス」, 「耐性菌保菌者の隔離」, 「カテーテル由来血流感染症の対策」, 「適切な洗浄・滅菌」, 「適切な抗菌薬使用のコントロール」, 「職員のインフルエンザ予防接種」などであった. また, 手術室やICU入室時の靴の履き換えなど, 現在必要とされなくなった感染制御も, 依然として高率に実行されていた. 効果的な病院感染制御の実施には, 感染制御専任者や専門的知識を持ったスタッフの確保を始めとする, マンパワーの問題を早急に解決する必要がある.
