Abstract

自己免疫性膵炎は高率に硬化性胆管炎を合併する．びまん性に硬化性の胆管狭窄をきたす症例では難治性の原発性硬化性胆管炎との鑑別を，限局性に胆管狭窄をきたす症例においては胆管癌との鑑別が重要である．自己免疫性膵炎に合併する硬化性胆管炎は膵病変と同様にステロイドに良好に反応するため，肝移植の適応となる原発性硬化性胆管炎と正確に鑑別することが大切である．そのためには臨床像，胆管像，超音波像などの画像診断および病理像の特徴を理解する必要がある．また膵病変をともなわず硬化性胆管炎単独で発症する症例もありこのような症例の診断は難しい．<br>
