Abstract

今回われわれは，陰茎亀頭部に限局した母斑の治療に人工真皮を用いて良好な結果を得たので報告する。症例は12歳，男性。下腿骨折にて入院中に，陰茎のかゆみにて泌尿器科を受診時，包皮を翻転した際に発見された。色素性母斑は12×15mmで亀頭部に存在した。辺縁は不整で，隆起はしていなかった。また陰茎包皮には母斑は認めなかった。全身麻酔下にSurgical Marginを１mmとり，皮膚を全層で切除したのち，人工真皮を縫合固定した。１週後にシリコンシートをはずした。３週後に創は拘縮なく自然閉鎖した。病理組織像はJunctional nevusで，surgical marginは陰性であった。比較的稀な分離母斑でない亀頭部に限局した色素性母斑に対して，人工真皮を用いた治療にて良好な結果を得た。術後６カ月経過して，色調は良好で瘢痕拘縮を認めていない。
