Abstract

Pneumatosis cystoides intestinalis (PCI) is a rare condition with numerous etiologies. PCI with intestinal ischemia is regarded as an ominous sign, because of the high mortality rate associated with this combination. We report a case of the gangrenous type of ischemic colitis showing PCI. A-80-year-old man was referred to us for investigation of ileus with progressively worsening abdominal pain and diarrhea. He had a history of diabetes, hyperlipidemia, and an old myocardial infarction. Physical examination showed abdominal swelling and tenderness in the upper abdominal site with rebound tenderness and muscular defense. An Abdominal X-ray and abdominal computed tomography showed intestinal dilatation with PCI from the ascending colon to the transverse colon. Under a suspected diagnosis of necrosis of the intestine and panperitonitis, emergency surgery was performed. A subtotal colectomy was performed for necrosis of the large intestine from the cecum to the descending colon. The postoperative course was uneventful. Although mortality from the gangrenous type of ischemic colitis is high, the immediate surgery successfully saved the life of this patient. In patients with the gangrenous type of ischemic colitis, early diagnosis and immediate surgery should be performed.
