We introduced a new technique to exchange a percutaneous gastrostomy button-type catheter under the guidance of transnasal gastroscopy with snare. During the procedure, the guidewire inserted through the gastric fistula was grasped endoscopially. Transnasal type was used in 10 patients, while peroral type in 10. We compared the two groups in terms of the procedure time, gagging episodes, and maneuverability. In patients undergoing transnasal endoscopy, the time was significantly shorter (6.6±0.7 min. vs. 8.9±0.7 min. mean±SD, P<0.001) ; gagging episodes were fewer (0.6±0.5 times vs. 2.3±0.4 times, mean ±SID, P<0.001) ; and insertion of the endoscopy was easier as compared to those undergoing peroral endoscopy. There was no complication in both groups. This technique using transnasal gastroscopy, guidewire and snare may be less invasive and safer.
