A CASE OF HERNIATION THROUGH THE FORAMEN OF WINSLOW DIAGNOSED PREOPERATIVELY ON MD CT

Abstract

A 65-year-old man complained of abdominal pain and vomiting. Abdominal CT scan showed a dilated intestine surrounded by the dorsal aspect of the left lobe of the liver, the lesser curvature of the stomach, and the hepatoduodenal ligament. Sagittal and coronal sections of the abdominal CT scan clearly demonstrated that the intestine had incarcerated in the space located between the portal vein and the inferior vena cava. The patient was diagnosed as having an intestinal obstruction due to an internal herniation via the foramen of Winslow. The patient had emergency surgery. On laparotomy, it was found that the ileum had become incarcerated about 40 cm in the omental bursa via the foramen of Winslow. Since the herniated bowel was viable, a manual reduction was performed without an intestinal resection. In this patient multi-detector CT was useful for making an accurate preoperative diagnosis of a Winslow foramen hernia. This entity should be considered in the differential diagnosis in patients with intestinal obstruction of unknown origin.
