Which is more accurate to measure for the axial length, with or without the mydriasis? We compared the situations by using two devices; the IOLMaster, which is a new optical biometry made by Carl ZEISS Company, and the US-800 A-mode, used as standard, made by NIDEK Company. The study involved 20 eyes of 10 normal subjects aging between 22 and 28-years old. We measured the axial length and the anterior chamber depth with and without the mydriasis by the IOLMaster and the A-mode devices in all subjects.<br>The result showed the significant differences among three groups of condition, which were the A-mode measurement with the mydriasis, the IOLMaster measurement with and without the mydriasis, against the condition of the A-mode measurement without the mydriasis. The study also revealed the IOLMaster had the significant differences in measurement of the anterior chamber depth with and without the mydriasis. The standard deviation of the axial length measurement and the anterior chamber depth was relatively small according to the IOLMaster in both conditions with and without the mydriasis, and was the largest according to the A-mode in the condition without the mydriasis. In addition, the standard deviation of measurement in the axial length and the anterior chamber depth according to the pupil size remained almost constant in IOLMaster. On the other hand, the A-mode measurement showed a larger standard deviation for a smaller pupil size, and the vice versa.<br>As a conclusion, we found the A-mode measurement was more accurate with the mydriasis, and the IOLMaster resulted in no significant differences with and without the mydriasis. From the above, the IOLMaster, a new optical biometry measurement of the axial length, is proven to be a quite useful device to use in the clinical setting.
