Nationwide breast cancer screening by mammography requires suitably qualified doctors to read the films. Short training courses on mammogram interpretation with quality assurance need to be held across the nation. We investigated whether the quality of a course, as required by the Central Committee for Quality Control, could be evaluated by questionnaire, rather than comparison of film readings before and after the courses held in Osaka and Kyoto as part of a research project (Chairman, N. Ouchi) subsidized by the Ministry of Health and Welfare in 1999. The proportion of doctors with experience in reading more than 201 mammograms was 81% in Osaka, 8 % in Kyoto B (beginners), and 60% in Kyoto E (experienced), respectively. The proportion of doctors who obtained A (instructor) level and B (adequate) level in film reading tests after the courses was 28% and 48% in Osaka, 5 % and 51% in Kyoto B, and 26% and 61% in Kyoto E, respectively. Doctors in Osaka and Kyoto E, where the proportion of experienced doctors was high, showed greater ability. Self-assessment of reading improved after the course in Osaka, but not in Kyoto. This may be attributed to the difficulty in understanding the Category Classification adopted recently, although participants indicated they did understand it. Most were aware of improvement in their ability and were sure that what they learned would be useful in future practice. The questionnaire method provides an easy assessment of the quality of a mammography training course, and should be used at each course.
