A case of a gastric bronchogenic cyst diagnosed concomitantly with a pancreas mucinous cystic neoplasm

Abstract

A 35-year-old man was found to have abnormalities in the cardiac region on an upper gastrointestinal series. Abdominal CT/MRI revealed a cystic lesion 5 cm in diameter in the pancreatic tail. The tail of the pancreatic duct was not depicted on MRCP ; thus, a, tumor could not be excluded. The monolocular tumor had a comparatively uniform mucin image on EUS. A presumptive diagnosis of a pancreas mucinous cystic neoplasm was made, and a distal pancreatectomy was planned. During surgery, it was found that the mass originated in the upper body of the stomach. Thus, a local resection was done. The cut surface of the excised specimen showed a unicameral cyst immediately under the gastric mucosa ; it contained a jelly-like substance. On histopathology, the cyst wall was lined by 1-3 levels of cube and cast skin cells. Mucin was present in the bulla body of the cyst epithelium and cilia of bronchial epithelium were seen in the lumen lining. Thus, a gastric bronchogenic cyst was diagnosed.
