Abstract

The separated orifice of the common bile duct from the pancreatic duct orifice (hereafter SO)is rarely encountered, and its clinical significance has not been disclosed. Three SO cases were examined. All orifice were located in the vicinity of the diverticulum,2 patients of them complicated choledocholithiasis. We performed endoscopic sphincterotomy (EST) and stones were removed completely after endoscopic mechanical lithotripsy (EML). Two patients with SO and nine patients without SO undergone endoscopic biopsy of the ampulla of Vater were retrospectively studied. Endoscopic biopsy showed that all SO patients had chronic papillitis in the background of plasma cell infiltration as well as hyperplastic mucosa. It may indicate that the chronic papillitis is attributed to the impaired bile outflow due to SO. These results suggest that SO cases could be the high risk group of choledocholithiasis. In cases of SO complicated choledocholithiasis, one should conduct EST with great care paying attention to the diverticulum in the vicinity of the ampulla of Vater.
