A 46-year-old man with ulcerative colitis and cholelithiasis was admitted for liver dysfunction. Endoscopic retrograde cholangiography revealed a beaded appearance of the common bile duct, cholecysto-choledocholithiasis and a pruned tree appearance of the intrahepatic bile duct. We diagnosed primary sclerosing cholangitis, and performed cholecystectomy with choledochotomy. After 26 months, the serum ALP value elevated. Endoscopic retrograde cholangiography showed irregular stricture of the lower bile duct, and computed tomography showed thickening of the wall of the common bile duct. Cytology of bile juice taken by endoscopic retrograde cholangiography was class III. Pyrorus-preserving pancreatoduodenectomy was performed on suspicion of bile duct carcinoma. The pathological diagnosis was sclerosing cholangitis. The patient is in good health 2years after resection.
