Cholangiocarcinoma treated by non-curative resection has generally a poor prognosis. We report a case with common bile duct cancer that underwent PPPD with positive surgical margin of hepatic-side bile duct eventually obtained a long-term survival after multimodality therapy. A 58year-old male had been aware of back pain. A series of examination diagnosed as common bile duct cancer, which was treated with PPPD. Although intraoperative pathology diagnosed the surgical margin of the bile duct as mild atypical cell, postoperative pathology finally found that the surgical margin of hepatic-side bile duct was positive. Thereafter, oral anticancer drugs were continuously administered. When hepatic metastasis was detected 3.5 years after the surgery, radio frequency ablation (RFA) was performed. When recurrence of anastomotic site of bile duct and jejunum was diagnosed 4.5 years after the surgery, a metallic stent was inserted at the anastomotic site under endoscopic guidance and chemoradiotherapy was applied as well. The recurrent lesion of anastomotic site of bile duct and jejunum did not show any enlargement and its tumor marker was in normal level. Nevertheless, the patient died of sepsis associated with hepatic abscess caused by cholangitis 5 years and 10 months after the surgery
