A 41-year-old man was admitted to the Critical Care Medical Center of Kinki University Hospital on October 23rd 1985,because of left abdominal pain spreading toward the left back. Physical examination of the abdomen demonstrated tenderness at the left epigastric region and splenomegaly. The ultrasonic and CT examinations of the abdomen showed intrasplenic and intrapancreatic fluid collections and ascites, leading the diagnosis of acute pancreatitis with pseudocysts. Because of pyrexia, leucocytosis and septicemia, transformation of the splenic cyst into abscess was suspected, and laparotomy and drainage of the content of the splenic cyst were performed on November 19th. We found the intrasplenic pancreatic pseudocyst containing gray-white pus of 600 ml infected with Staphylococcus aureus with the amylase activity of 427,000 IU/l. Postoperatively, the patient made uneventful progress and was discharged. Twenty-two cases of intrasplenic pancreatic pseudocyst including our case above mentioned in Japanese literature were briefly reviewed.
