A report was made of a case of cerebrovascular accidents who developed illusive metamorphopsias and interesting EEG findings. This 69-year-old man was admitted to Kinki University Hospital for the first time because of convulsion and metamorphopsias. CT scans showed a lesion adjacent to the middle temporal gyrus in the right hemisphere. Metamorphopsias were mainly teleopsia and pelopsia which were complex illusions of Hecaen-Albert classification. It was considered that this appearance of symptoms was caused by mass effect in the occipital areas. EEG showed peculiar periodic localized 4-5 Hz theta wave with 80-90 microvolt in the lesional area. Photic stimulation, hyperventilation and opening and closing of eyes did not have any influence on this theta wave. With the improvement of clinical symptoms, this theta wave disappeared.
