A 67-year-old man was admitted to our hospital because of anterior chest pain and dyspnea, on February 4,1987. On examination, rales were audible in both lungs. A hash grade III/VI pansystolic murmur, which radiated to the left axilla, was heard at the apex. The chest roentgenogram showed severe pulmonary congestion. The electrocardiogram revealed evidence of acute lateral infarction. Mitral regurgitation was demonstrated by the Doppler echocardiography. The pulmonary artery wedge pressure tracing revealed a huge "V" waves. On the seventh hospital day, however, he died of acute renal failure. The postmortem examination revealed lateral wall necrosis of the left ventricle and rupture of the anterior papillary muscle. Histological findings showed hemorrhage of the papillary muscle. Before death, papillary muscle rupture was suspected.
