A pseudomalignant erosive and ulcerative lesion occuring in a 67-year-old woman is reported. A routine barium meal study of the upper gastrointestinal tract revealed a polypoid lesion in the lower esophagus associated with mild functional insufficiency of the cardia. Endoscopically, the lesion was erosive and lobulated. The first endoscopic biopsy specimen was considered suspicious of malignant neoplasm. Endoscopic polypectomy was then performed and the histologic examination also suggested morphologic malignancy, although immunohistologic staining showed no apparent features of differentiation except for positive staining for vimentin. However, in the follow-up studies with repeated endoscopic examinations and biopsies, there were no atypical cell has been found. Thus, the patient has been free from any malignant change for thirteen months since the first biopsy. Based on these endoscopic features, histopathologic findings and follow-up data, we conclude that the polyp was most probably a pseudomalignant erosive and ulcerative lesion. The present case suggests that understanding the clinicopathologic features of this lesion is indispensable for endoscopists to avoid unnecessary surgery.
