  • Changes in the Consultation Rate Incidence of New Cases, and Mortality Rate for Chronic Bronchitis with Those in the State of Air Pollution in Yokkaichi Region

抄録

Correlation analysis was performed on the relationship of changes in air pollution in Yokkaichi City by taking into consideration the effects of various socioeconmic factors. The relationships of the incidence of new cases and mortality rate due to chronic bronchitis with the trend of air pollution were also evaluated. The consultation rate of middle-aged and aged individuals (40 years and above) for chronic bronchitis tended to decrease for the first time more than 20 years after the state of pollution began to improve in this region, but there has consistently been a significant correlation between the consultation rate and the degree of air pollution. Partial correlation coefficients of the consultation rate for chronic bronchitis with the degree of pollution, socioeconomic factors, and the rate of heavy smokers were calculated. As significant correlation was observed between the consultation rate and the family size. When the polluted area was compared with the nonpolluted (control) area, the consultation rate was consistently higher in the polluted area than in the reference area (residual effect), and the statistical significance of the difference continued, partly because of the refractoriness of chronic bronchitis once it is established. However, the incidence of new cases of chronic bronchitis in the age group 40 years and above decreased to the level of the reference area in about 4 years, closely reflecting alleviation of air pollution. Also, the mortality rate due to chronic bronchitis increased in all age groups with a decrease in pollution in the polluted area but decreased to the level of the control area with a delay of 4-5 years after the sulfur oxide concentration was reduced below the environmental standard. Recently, it has become comparable to that in the control area and has remained stable. Although the consultation rate of middle-aged to aged patients with chronic bronchitis in the polluted area tends to decrease, it has remained sightly higher than that in the control area due to residual effect. However, the incidence of new cases has already become similar to that in the control area, and the difference in the consulation rate is also expected to disappear in the near future.
