抄録

Background: As an activity of the “Anti-Lung Cancer Association”, charged periodical medical examination for lung cancer has long been conducted according to a membership system whereby chest X-ray film and sputum cytology have been applied every 6 months. Nine years have now passed since the inclusion of helical computed tomography (CT) in this examination. Five patients with the peripheral type of squamous cell carcinomas were detected during this period, for whom atypical squamous cells had been evaluated as level D or greater by sputum cytology prior to CT detection. This result underscores the importance of sputum cytology for the detection of the peripheral type of squamous cell carcinoma. The sequential changes in sputum cytology and CT findings are reviewed for these 5 patients.<BR>Cases: All the 5 patients were men, and the age at the time of initial detection of cancer ranged from 65 to 79 years. The smoking history ranged from 30.0 to 102.5. In all the cases, squamous cell carcinoma was detected 1.5 years to 4.5 years after the appearance of atypical squamous cells evaluated as level D or greater by sputum cytology. Re-examination of CT images revealed no abnormal findings in one case. In the other 4 patients, 0.5-1.5 years before the final detection of cancer, shadows were observed at the corresponding sites which were not recognized as cancer.<BR>Conclusions: When atypical squamous cells, which are evaluated as level D or greater by sputum cytology, appear even in the absence of abnormal findings on CT, continuous testing, evaluation of more detailed images, and close observation of the course are important in view of the potential existence of minute peripheral squamous cell carcinomas.
