Background: Papillary renal cell carcinoma is an uncommon subtype of renal epithelial neoplasm with distinctive morphological and radiological features. Only a few reports have described its cytological features.<BR>Case: A 42-year-old woman was admitted for evaluation of prolonged loss of her appetite and body weight. Abdominal ultrasonography disclosed a large mass in the left kidney. Angiography revealed a hypovascular tumor. The patient underwent radical nephrectomy. A large necrotic tumor, measuring 15×10×10cm, involved a large part of the left kidney. Aspiration cytology showed cell clusters with papillary structures and tadpole-shaped or racquet-shaped isolated cells. Histology suggested papillary renal cell carcinoma. An immunohistochemical and ultrastructural study suggested this tumor originated from the distal tubular epithelium.<BR>Conclusion: Tadpole-shaped or racquet-shaped isolated cells with cell clusters showing papillary structures may be a cytological characteristic of papillary renal cell carcinoma.
