Background: Invasive micropapillary carcinoma (IMC) of the breast is a recently described variant of invasive ductal carcinoma, characterized by the formation of micropapillae within clear spaces separated by fibrocollagenous stroma. IMC extensively invades lymph vessels and involves a high incidence of lymph node metastasis. Cytological findings of IMC remain poorly defined.<BR>Case: A 65-year-old woman referred to us due to a left breast tumor noticed several years ago was found in aspiration cytology to have moderately cellular smears composed of oval to angulated, 3-dimensional neoplastic cell clusters in a clear background. Cell clusters had a papillary configuration without a fibrovascular core. The excised breast tumor consisted primarily of typical IMC and had metastasized to 5 of 14 lymph nodes.<BR>Conclusion: A case of IMC is presented with cytological findings.
