Aspiration cytology in cystic conditions of the salivary gland have low diagnostic accuracy because of frequent false negative cases and low-grade malignant tumors. We reviewed such cases, and divided cystic conditions into 4 categories-mucus-rich, hyalin materials, basalioid cell, and squamous cells. We discuss differential diagnoses and diagnostic pitfalls in each category. If materials were insufficient for diagnosis, reaspiration of solid areas or subsequent reaspiration should be done. In such cases, we should not report “no malignancy”. It was difficult to diagnose lowgrade mucoepidermoid carcinoma, even if tumor cells were aspirated. In cystic conditions, it is important to observe all materials in detail, especially the background, and to be aware of diagnostic pitfalls. Cystic conditions associated with inflammation and/or an unusual solid mass and arising from ectopic salivary glands should be also noted as pitfalls.
