A 71-year-old man was referred to our hospital for the evaluation of an enlarged cavitary lesion in the left lung and treatment of left pneumo thorax. Chest CT taken 4 months before the operation showed a thin-walled cavitary lesion, 1.5 × 1.8 × 2.0 cm in size, in the peripheral region of the left S3 and left localized pneumothorax.<BR>After the diagnosis of malignancy, left upper lobectomy, decortication of the left lower lobe and lavage of the left pleural space with MMC 20 mg were performed.<BR>A thin-walled cavity of the primary lung cancer may be caused by ; 1) necrosis, liquefaction and drainage ; 2) check-valve mechanism ; 3) elastic traction of the pulmonary tissue ; 4) infiltration of cancer cells to the pre-existing cavity. The rupture of the cavitary lesion may be caused by ; 1) necrosis of the cavity wall ; 2) check-valve mechanism ; 3) direct pleural invasion of the tumor ; 4) rapid atmospheric pressure change.<BR>Considering the radiological findings and pathological examination of this case, we thought that the thin-walled cavity was produced chiefly by the check-valve mechanism and the rupture of the cavitary lesion was caused by the combined effect of the direct pleural invasion, necrosis of the cavity wall and check-valve mechanism.
