A 71-year-old man was referred to our hospital for the evaluation of an enlarged cavitary lesion in the left lung and treatment of left pneumo thorax. Chest CT taken 4 months before the operation showed a thin-walled cavitary lesion, 1.5 × 1.8 × 2.0 cm in size, in the peripheral region of the left S3 and left localized pneumothorax.
After the diagnosis of malignancy, left upper lobectomy, decortication of the left lower lobe and lavage of the left pleural space with MMC 20 mg were performed.
A thin-walled cavity of the primary lung cancer may be caused by ; 1) necrosis, liquefaction and drainage ; 2) check-valve mechanism ; 3) elastic traction of the pulmonary tissue ; 4) infiltration of cancer cells to the pre-existing cavity. The rupture of the cavitary lesion may be caused by ; 1) necrosis of the cavity wall ; 2) check-valve mechanism ; 3) direct pleural invasion of the tumor ; 4) rapid atmospheric pressure change.
Considering the radiological findings and pathological examination of this case, we thought that the thin-walled cavity was produced chiefly by the check-valve mechanism and the rupture of the cavitary lesion was caused by the combined effect of the direct pleural invasion, necrosis of the cavity wall and check-valve mechanism.
