A 66-year-old-man was admitted to our hospital for obstructive jaundice. Histological examination of a biopsied specimen of the papilla of Vater showed adenocarcinoma, and pylorus-preserving pancreaticoduodenectomy was performed. An ulcerated tumor (2.6×1.8cm) found in the papilla of Vater was adenoendocrine cell carcinoma. Histological findings showed adenocarcinoma component in mucosa area. Immnohistochemical findings showed positive staining of CEA, EMA, NSE both in endocrine cell carcinoma component and adenocarcinoma component. A transition between both components was seen histologically. It was suggested the same origin of the two components. The prognosis of endocrine cell carcinoma is very poor because of its rapid growth, metastasis, and invasion. Ten months after the operation, the patient had bone and paraaortic lymph node metastases. To the best of our knowledge, this is the fourth case ever reported of endocrine cell carcinoma of the papilla of Vater in Japan.
