There is usually a much better relationship between the pharmacologic effects and drug concentration in plasma rather than its dose. Individual adjustment of drug dosage on the basis of a targeted drug concentration in plasma may prevent adverse effects owing to inadvertent overdosage, and it may facilitate therapy by providing an alternative to the titration of dosage on the basis of therapeutic response or toxic effects. These considerations have led to the extensive use of therapeutic drug monitoring (TDM) in recent years. So far, TDM has been focused primarily on the pharmacokinetic aspects of the individualized optimization of drug dosage. However, it has become evident that there can be appreciable inter-individual differences in pharmacodynamics. In the present paper, pharmacodynamic aspects of TDM are reviewed with some practical case reports on propranolol, theophylline, procainamide, digoxin and zonisamide. With learning the effects of individual characteristics on the relationship between drug concentration and pharmacologic activity in more detail, it will become possible to make reasonable predictions and to combine this information with our pharmacokinetic data base.
