臍帯ヘルニアに膀胱臍瘻 (尿膜管無形成) を合併した1例 : その発生機序について  [in Japanese] A Case of Hernia into the Umbilical Cord Associated with Vesico-Umbilical Fistula  [in Japanese]

尿膜管異常の中でも膀胱臍瘻は比較的稀な病態であるが, 我々は膀胱粘膜が外反脱出した膀胱臍瘻を合併した臍帯ヘルニアの1例を経験した. 超音波断層による妊娠経過の観察より, 病態の発生機序として以下が考えられた. 初めに膀胱に連続した大きな cyst が臍帯内に形成され, 臍輪の形成が障害された. その後, 臍帯内の cyst が羊膜腔に破れ, cyst は縮小して膀胱とつながった瘻孔が形成された. cyst の縮小に伴い拡張した臍帯内に腸管が侵入し, hernia into the umbilical cord となった. Hernia into the umbilical cord は腸管の腹腔内還納異常によると言われているが, 本病態での hernia into the umbilical cord は通常の発生とは機序が異なると推察された.治療はヘルニア嚢と共に瘻孔を含めて膀胱頂部まで切除して膀胱を閉鎖し, 腹壁を閉鎖した. 術後は原因不明の回腸穿孔を起こしたが事なきを得, 泌尿器系の合併症はなく経過している.

Vesico-umbilical fistula is a rare disorder among anomalies of the urachus. We successfully treated a newborne infant who had a hernia into the umbilical cord assosiated with vesico-umbilical fistula. The serial antenatal sonographic imaging allowed us to hypothesize that rupture of the large cyst existed in the umbilical cord and communicated with the urinary bladder caused secondary prolapse of the intestine into the enlarged cord that resulted in the small omphalocele.
