We report here a case of extrauterine endometrial stromal tumor in a 39-year-old Japanese woman. Laparotomy revealed a man's fist-sized cystic mass located retroperitoneally and adjacent to the uterine cervix and extending in front of the left parametrium. A total hysterectomy and bilateral salpingooophorectomy was performed. A fleshly mural nodule was included in the hemorrhagic cyst. No evidence of uterine tumor was noted. The tumor was characterized histologically by a monotonous proliferation of round or oval cells, widely distributed small vascular vessels, mild cellular atypia and low mitotic figures. Endometriotic foci were found but they were not associated with the neoplasm. Although immunohistochemically α-smooth muscle actin was found to be diffusely expressed in tumor cells, no bundles of filaments were ultrastructurally observed in the cytoplasm. The patient is well without recurrent disease 12 months postoperatively.
