Progressive Myelopathy Caused by Dural Arteriovenous Fistula at the Craniocervical Junction : Case Report

Abstract

頭蓋頚椎移行部に発生した硬膜動静脈瘻(dural AVF)の症例について報告した。患者は68才男性で下肢より始まり次第に上行する進行性のミエロパチーを呈した。MRIではC1, C2レベルで拡張した静脈が脊髄周囲に認められ、また髄内にT2強調像で高信号域を呈する所見が見られた。血管撮影では右椎骨動脈が硬膜を貫通する部位においてAVFが認められた。本症例におけるAVFは硬膜にnidus様の所見を有さず椎骨動脈と脊髄静脈との間のdirect fistulaの形態を呈していた。硬膜内静脈側で外科的にfistulaの遮断を行ったが、術後患者の神経症状は著明に改善した。

A 68-year-old male presented an unusual dural arteriovenous fistula (AVF) located at the craniocervical junction. Magnetic resonance imaging revealed dilated perimedullary veins around the spinal cord at C-1 and C-2 levels, as well as high intensity signals in the spinal cord on T_2-weighted images. Vertebral angiography identified an AVF at the point where the right vertebral artery penetrates the dura. The fistula was a single and direct communication between the vertebral artery and the spinal vein. Surgical interruption of the fistula at its venous side resulted in prompt improvement of both motor and sensory signs and symptoms.
