The authors experienced a case of small cell lung carcinoma with the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) and Lambert-Eaton myasthenic syndrome (LEMS). A 69-year-old male came to Asahikawa Red Cross hospital with complaints of hemoptysis and dyspnea. His chest X-ray revealed a tumor mass in the left upper lung field. Also hyponatremia was found. A diagnosis of small cell lung carcinoma was made by bronchoscopic biopsy. Sodium of the serum and the urine, osmolality of the plasma and the urine and renal function revealed persistent hyponatremia due to SIADH.<BR>The patient was active after starting combination chemotherapy but was readmitted due to general fatigue, dry mouth and weakness of the extremities. Electromyogram showed a waning phenomenon on low frequency stimulation, a waxing phenomenon on high frequency stimulation and post-tetanic facilitation. A diagnosis of Lambert-Eaton myasthenic syndrome was made on the basis of these electromyographic studies. Combination chemotherapy slightly improved the abnormal shadow but the muscle weakness remained unchanged. The patient died 3 months after readmission due to rupture of a dissecting aneurysm of the thoracic aorta.
