From 1971 to 1994, 11 patients with adenoid cystic carcinoma of the central airway underwent surgical resection in our institute. Bronchoscopic findings of these tumors were very characteristic and preoperative diagnoses were obtained in all cases by transbronchial biopsy. Bronchoscopic findings correlated well with tumor invasion in the tracheobronchial wall and lung parenchyma. In 4 cases of adenoid cystic carcinoma of the trachea that received Nd: Yag laser treatment preoperatively, effective relief of clinical symptoms due to airway stenosis increased therapeutic options. Tracheobroncoplasty was effective method for the surgical treatment of these tumors but surgical margins were positive in most cases. Postoperative radiotherapy was performed for these cases and good local control was obtained with 60 Gy irradiation. The surgical outcome of these cases was generally good and the postoperative 5-year survival was 100%, excluding one case of operative death. The mean survival of these cases was 89.8 months.
