Background: Myelodysplastic syndrome (MDS), defined as a preleukemic clonal abnormality of hematopoietic stemcells, is found frequently concomitant with non-hematologic malignancy. Cases of MDS developing secondary to chemotherapyfor lung cancer are often reported, but lung cancer concomitant with primary MDS is rare.<BR>Case: MDS was diagnosed by bone marrow examination in a 77-year-old woman with anemia and a low white blood cellcount. At the same time, chest roentgenogram showed a tumor in the right lower lobe. The tumor was diagnosed by CTguidedlung biopsy as stage IA (c T1N0M0) lung adenocarcinoma.<BR>Right lower lobectomy and mediastinal-hilar lymph node dissection were carried out without any complication. Histologicexamination of surgical specimens showed metastasis of a small nodule near the tumor and mediastinal lymphnodes (p T3N2M0, stage III B). Adjuvant therapy was avoided because of her age and possible complications of chemotherapy. This patient is still alive one year after surgery.<BR>Conclusion: Resected lung cancer with primary MDS is extremely rare, and this case is the fifth report in the literature.
