Background: Leiomyosarcoma of the uterus is a rare disease and its prognosis is poor. Neither chemotherapy nor radiotherapy is effective. Since lung tumor metastatic from uterine leiomyosarcoma usually grows rapidly, its early detection and surgical resection are crucial for the treatment.<BR>Case: A 57-year-old asymptomatic woman was admitted to our hospital to examine abnormal shadows detected on chest X-ray films. Leiomyosarcoma of the uterus had been diagnosed 33 months previously and she had undergone hysterectomy and oophorectomy with subsequent systemic chemotherapy. There were two round metastatic lesions in the right lung (S2 and S10). After confirming no other distant metastases or local recurrence, resection of the tumors (segmentectomy of the right S2 and wedge resection of the S10 tumor) were performed. The size of the tumors in right S2 and S10 were 22×18×25mm and 12×11×13mm respectively. Pathological examinations revealed smooth muscle cells with nucler pleomorphism and high mitotic indices. Immunohistochemically, the resected specimen showed positive smooth muscle actin and vimentin staining, but was negative for S-100 protein and keratin. The tumors proved to be lung metastases of uterine leiomyosarcoma. No evidence of disease recurrence was found during the five-month postoperative period.<BR>Conclusion: We report on 57-year-old woman with lung tumors metastatic from uterine leiomyosarcoma. Resection of lung metastases is the treatment of choice as long as other distant metastasis is not identified.
