Background. The incidence of thymic carcinoma is relatively rare compared with thymoma. Severalregimens of platinum-based chemotherapy have been tried for the treatment of advanced thymic carcinoma, however, there still has been no consensus on the standard regimen. Case. A 64 year-old man visited our institution complainingof dyspnea. Chest X-ray and CT films demonstrated a mass in the left anterior mediastinum and diffuse nodularthickening of the pleura accompanied by effusion. MR imaging revealed that the lesion extended over the diaphragm tothe retroperitoneal tissues. Microscopic examinations of specimens obtained by CT-guided needle biopsy revealed proliferatingatypical cells with pleomorphism and necrosis. Overall findings were compatible with thymic carcinoma, stage IVa. Chemotherapy using carboplatin (AUC 5mg/ml·min, day 1) and docetaxel (60mg/m2, day 1) was started.Theresponse was prominent even after one course of treatment, and complete response was finally obtained after fourcourses. The serum level of soluble IL-2 receptors decreased from 8, 330 to 730 after the treatment. Conclusion. Chemotherapyusing carboplatin (or cisplatin) and docetaxel may be a promising regimen for the treatment of thymic carcinoma.
