The clinical and histologic characteristics of 15 pulmonary adenocarcinomas showing intraluminal polypoid growth were studied. The incidence was 8.0% out of 188 adenocarcinoma cases with pathological findings on fiberoptic bronchoscopy. Six cases (40%) were asymptomatic and detected by abnormal chest X-ray. The most common symptom was bloody sputum. The majority of cases showed a relatively well-defined mass shadow on chest X-ray film and 10 cases (67%) were accompanied by secondary changes. Bronchial brushing cytology and transbronchial biopsy had high diagnostic yields of 87% and 80%, respectively. However, sputum cytology provided a definite diagnosis in only 31% of the cases. The prognosis was generally unfavorable in both resected and non-resected cases.<BR>Histologically, tubular adenocarcinoma was more common than papillary adenocarcinoma. This type of adenocarcinoma was classified either as bronchial gland cell type or bronchial surface epithelial cell type according to cell subtyping.<BR>From these clinical and histological features, it is considered that adenocarcinoma showing intraluminal polypoid growth is a unique subgroup of pulmonary adenocarcinoma.
