The clinical and histologic characteristics of 15 pulmonary adenocarcinomas showing intraluminal polypoid growth were studied. The incidence was 8.0% out of 188 adenocarcinoma cases with pathological findings on fiberoptic bronchoscopy. Six cases (40%) were asymptomatic and detected by abnormal chest X-ray. The most common symptom was bloody sputum. The majority of cases showed a relatively well-defined mass shadow on chest X-ray film and 10 cases (67%) were accompanied by secondary changes. Bronchial brushing cytology and transbronchial biopsy had high diagnostic yields of 87% and 80%, respectively. However, sputum cytology provided a definite diagnosis in only 31% of the cases. The prognosis was generally unfavorable in both resected and non-resected cases.
Histologically, tubular adenocarcinoma was more common than papillary adenocarcinoma. This type of adenocarcinoma was classified either as bronchial gland cell type or bronchial surface epithelial cell type according to cell subtyping.
From these clinical and histological features, it is considered that adenocarcinoma showing intraluminal polypoid growth is a unique subgroup of pulmonary adenocarcinoma.
