A 50-year-old male was admitted because of cough. Chest X-ray film and CT scan showed anterior mediastinal tumor and needle biopsy yielded a diagnosis of poorly differentiated squamous cell carcinoma. Abdominal CT scan showed solitary liver tumor. Arterial injection of CDDP and ADM and TAE was performed. Following irradiation to the mediastinum, a decrease in tumor size was confirmed by CT scan. Despite thoracotomy, the tumor was unresectable because of widespread tumor invasion to the aorta, superior vena cava and right atrium. The pathological diagnosis was mixed small cell undifferentiated squamous cell carcinoma. Postoperative chemotherapy with CDDP was performed. Four months after thoracotomy, chest wall resection was performed for radiation ulcer of the anterior chest wall and biopsy of the tumor showed complete response to chemotherapy and radiotherapy pathologically. Additional chemotherapy with CDDP was performed for liver and lung metastasis. Although the lung metastasis disapeared, the patient died 39 weeks after thoracotomy. Further experience with combination chemotherapy and radiotherapy is necessary.
