Abstract
In 1999, the Institute of Medicine (IOM) of the USA published "To Err Is Human" in which they recommended 5 principles for building a patient safety system : 1. Provide Leadership, 2. Respect Human Limits in Process Design, 3. Promote Effective Team Functioning, 4. Promote Effective Team Functioning, and 5. Create a Learning Environment. We reviewed them and found that these principles were global for patient safety issues. We tried to apply them to our hospital in these years, and were able successfully to improve our system in Japan.
