Abstract
当施設のある青森県津軽地域は, 脳神経外科施設が少なく, 脳神経外科的治療の必要な症例は地域基幹病院から当施設などに遠距離搬送を行う必要がある. また当地域は冬期の積雪量が多く, 気候が厳しいことで知られているが, 患者搬送の障害になることも多い. 私たちは当地域の基幹病院との間で, 脳卒中や頭部外傷などの脳神経外科疾患に対して画像電送システムを用いた遠隔医療を行ってきた. 診断, 手術適応など外科的治療の必要性の観点から, 症例ごとに患者搬送の適否を検討している. 本論文では当施設で行っている画像電送装置を用いた遠隔医療によって紹介されたくも膜下出血症例の臨床的特徴や治療成績について検討したので報告する. 対象と方法 画像電送にはPhotophone WS型(Imagedata Co. Ltd., USA)を用いた(Fig. 1). 本システムは青森県津軽地域の計11病院と通常の電話回線で結ばれており, 24時間症例の相談や紹介を可能とした(Fig. 2).
Telemedicine offers potential advantages for regional healthcare. We report the usefulness of telemedicine using an image transfer system in the treatment of aneurysmai subarachnoid hemorrhage. An image transfer system, installed in our hospital and 11 regional hospitals in the Tsugaru district of Aomori Prefecture, has been utilized for teleconsultation of neurosurgical patients presenting with stroke, head trauma, and others disorders, since 1989. Neuroimaging studies such as computed tomography were transmitted from the emergency care units of local hospitals to our department. Most subarachnoid hemorrhage patients were transported and treated by direct surgery or embolization. The group of patients referred using this system included cases that were less severely graded than those in the group of patients directly transported to t,he emergency unit of our hospital. The final outcomes of the referred patients were as favorable as those of the directly transported patients. Telemedicine using an image transfer system was sufficient for the care of neurosurgical cases, including subarachnoid hemorrhage patients.
