This study examined the level of secretory leukocyte protease inhibitor (SLPI) and α1-protease inhibitor (α1-PI) in gingival crevicular fluid (GCF) of adult periodontitis patients. Fifteen periodontitis sites (probing depth>4mm, clinical attachment loss>4mm, positive bleeding on probing) and healthy sites (probing depth<3mm, clinical attachment loss<3mm, negative bleeding on probing) were selected from fifteen periodontitis patients after completion of oral hygiene. The levels of SLPI and α1-PI in GCF were determined with a commercially available ELISA system. Neutrophil elastase (NE) activity in GCF was measured with the low molecular weight substrate S-2484. The results were expressed as absolute amounts per sample. Significantly higher amounts of SLPI (p=0.01), α1-PI (p=0.001), NE (p<0.005), and GCF (p<0.001) were consistently obtained in samples from periodontitis sites than in samples from healthy sites. SLPI (rs=0.4), α1-PI (rs=0.697) and NE (rs=0.69) were found to correlate with probing depth. NE andα1-PI were significantly correlated with increasing clinical attachment loss. These results suggest that SLPI and α1-PI may be related to the inflammation in periodontitis sites. In addition, larger amounts of protease inhibitors were released in periodontitis sites than in healthy sites, whereas the protease inhibitors cannot inactivate NE. J. Jpn. Soc.
